Case study – Supporting a client’s journey home

After experiencing problems with their existing staffing provider, a case manager called the Scottish Nursing Guild for support in securing a safe hospital discharge for their client. A road traffic accident had left them with life-changing injuries, leaving him paralysed and requiring long-term care.

Collaborating with the case manager, our nurses ensured a smooth transition home while the long-term care plan was being shaped. The Guild’s support remained consistent, providing the client with experienced nurses and healthcare assistants to support with clinical, personal, and social care until the new provider’s staff were inducted, and we continued to fill gaps in the care package as needed.
